Joshua Tree Home is implementing the CAPABLE program, developed by Johns Hopkins School of Nursing. CAPABLE aims to enable older adults to remain in their homes longer, achieve better health outcomes, and reduce medical costs.

CAPABLE is a person-directed, home-based program designed to enhance independence while reducing healthcare costs. Over four to five months, an occupational therapist (OT), a registered nurse (RN), and a handy worker collaborate with older adults to set goals and implement action plans that promote health, safety, and functional improvements. Participants gain new skills, practice exercises, and learn to use tools, equipment, and home modifications to enhance daily living. By focusing on prevention and problem-solving, CAPABLE empowers individuals with lasting strategies for greater independence and well-being.
